Full-Time Organizational Case Manager

 SALARY:  $38,400,360 to $42,240 per year, 40 hours per week (1.0 FTE), includes medical, dental, vacation, sick leave, and holidays. 

POSITION DESCRIPTION

Under the supervision of the Assistant Director of the Glover EmpowerMentoring Program.  Provide an array of case management services to up to 40 youth at a time who are referred to GEM through its various point of entry.

ESSENTIAL FUNCTIONS

§  Manage a caseload of up to 40 (a minimum of 20 for the first three months) clients who are currently involved or at-risk of involvement with the juvenile justice/legal systems, gangs, and/or academic failure. 

§  Work as part of a team with other case managers, mentors, and outreach workers to recruit youth and young adults up to the age of 24 years of age to develop case load.

§  Focus services on reducing juvenile justice and legal system involvement, gang involvement, reducing school dropout, increasing pre-employment skills, and increasing employment opportunities.

§  Conduct home visits, provide case management, individual and group counseling, referral, follow up, and family support. Involves direct communication with clients and parents.

§  Contact and/or visit probation officers, juvenile courts, teachers, and other related agencies. 

§  Complete thorough assessments and create individualized service plans for caseload youth/young adults. 

§  Maintain case files, evaluation records, computerized information systems (as necessary), and other related paperwork. 

§  Work collaboratively with GEM team members, schools, existing agency partners, and team members in similar programs. 

§  Conduct outreach into the community to advocate on behalf of participants and programs. 

§  Supervise and be responsible for the general safety of clients in program functions and activities. 

§  Attend team meetings, trainings, conferences, and other similar events as needed. 

§  Perform other duties as necessary within a smaller organization.

§  This position will require some work during evenings and occasional weekends.

REQUIRED QUALIFICATIONS

Two years of direct social service experience with low income, at-risk youth and families (which includes experience involving interviewing, counseling, and providing crisis intervention with at-risk youth); and/or experience providing any of the essential functions listed above; AND a Bachelor’s Degree in social work, counseling, education or related social services field (Master’s Degree preferred or a combination of education and/or training and/or work experience which provides the ability to perform the essential functions of the position). Valid WSDL and safe driving record. Must be submitted within 30 days of hire.

DESIRED QUALIFICATIONS

Ability to gather, analyze, and use data to create, manage, and evaluate various programs to meet the needs of youth. Demonstrated ability to develop and operate youth peer groups and activities; ability to relate well with youth at-risk and take leadership roles with youth; ability to develop and implement pro-social skill building activities; knowledge of Asset Development model; knowledge of different cultures and refugee/immigrant history and relevant issues which impact the minority community; knowledge of the juvenile justice system and the public school system; extensive knowledge of alcohol/drug, mental health, and youth gang issues; knowledge and life experience within the communities served; strong written and oral communication skills.  Bilingual helpful.
